When you plug in both speakers and headphones into a Sound Blaster, the card takes the output sound and just splits it between the two.

What worked for me is changing the output source FFXIV uses. My headset has a game mode and chat mode, and it outputting to the chat mode produced a horrible static and caused the game to crash in directx 11. Switching it to game mode fixed all those issues and let me launch normally. 3.
